Dental Practice Group

New Patients must allow a comprehensive dental exam with complete set of X-rays. Arrive thirty minutes early to allow time to find parking and to fill out all necessary paperwork. We are located on the third floor of the Dental School. Just take a left off the elevators and follow the signs to the Dental Practice Group. These patients do NOT need to check in the lobby at the business office.

Endodontics

Fax 706-721-8184 - Referral required

Referrals must be faxed to the Endodontic Department from the referring dentist before an appointment can be scheduled. The fax should include the patient’s name, address, contact phone number, date of birth, and tooth number to be treated. When the fax is received, the patient will be contacted and an appointment will be made. Located in Clinic #6 on the second floor.

General Practice Residency Attendings

Oral Medicine

Referrals must be faxed to the Dental Practice Group from the referring dentist or physician before an appointment can be scheduled. The fax should include the patient’s name, address, contact phone number, date of birth, and tooth number to be treated. When the fax is received, the patient will be contacted and an appointment will be made. Located in Clinic #15 on the third floor. Go to the right off the elevator through Clinic #14 and through the double doors to the patient waiting area.
